How Many Peptides Can Be Stacked? What the Trials Combined

How many peptides are stacked in trials and approved products: a registry count of 101 trials, the two-peptide ceiling in every label, and four-peptide blends.

The question has a number in it, and no study supplies one. What the record can supply is how many peptides have actually been given together where someone measured the result — in registered trials and in approved products — and how that compares with the blends and stacks sold for research use. This page counts it. On 2026-10-20 this site searched ClinicalTrials.gov for 14 compounds sold on the research market and read the interventions of every study returned; it also checked which approved products combine peptides. The answer is short: two, in every case that was tested. It reports that count; it does not suggest a number for anyone.

The individual combinations are worked out on the peptide stacks hub, including the evidence and units for the muscle-growth stacks and the weight-loss stacks.

The registry count

What was searched Result
Registered studies naming any of the 14 compounds as an intervention 101
Studies giving two or more peptides (incl. the GLP-1 drugs) 31
Of those, sponsored by Novo Nordisk and testing cagrilintide with semaglutide 31 of 31
Studies combining any two of the other 13 compounds 0
Studies giving three peptides in one arm 0

The 14 compounds were BPC-157, TB-500, GHK-Cu, CJC-1295, ipamorelin, KPV, epitalon, sermorelin, tesamorelin, MOTS-c, AOD-9604, Selank, Semax and cagrilintide; a study counted as a combination when two named peptides appeared among its interventions. Three of the 31 also name tirzepatide, as a comparator arm rather than part of the combination. The one study with three active peptides, NCT06409130, gave them as two pairs in separate arms. The method has a known limit: a combination registered under a code name the search does not match would be missed.

Approved combinations are pairs

Two approved products put two peptides in one injection, and both are fixed-ratio pens for type 2 diabetes: XULTOPHY 100/3.6 combines insulin degludec with liraglutide (openFDA label effective 2025-10-14), and SOLIQUA 100/33 combines insulin glargine with lixisenatide (effective 2026-03-18). The fixed ratio is the defining feature — each unit dialled delivers both drugs in a set proportion, so neither can be adjusted alone. No approved product found combines three peptides.

The one research-market combination with a trial programme

Cagrilintide is sold on the research market and has been studied mostly in combination. Novo Nordisk's CagriSema programme pairs it with semaglutide, and the registry holds 31 studies of the pair: the REDEFINE phase 3 trials, regional trials in China and East Asia, kidney, appetite, bone and muscle studies, a paediatric study, and three trials against tirzepatide. Two are the kind of study a stack of any size would need and none has: a comparison of blood levels when the two drugs are given together and apart (NCT04940078), and a drug–drug interaction study with atorvastatin and warfarin (NCT06289504). The doses and results are on the cagrilintide and GLP-1 stack page.

What the two-peptide trials found

The CagriSema results are the only measured answer in this set to what adding a second peptide does, and they cut both ways. As this site's weight-loss stack page records them (sources read 2026-09-18):

Trial Who Result at end Gastrointestinal adverse events
REDEFINE 1 (NCT05567796; Garvey et al., NEJM 2025) 3,417 adults without diabetes, 68 weeks 20.4% mean weight change vs 3.0% on placebo 79.6% vs 39.9%
REDEFINE 2 (NCT05394519; Davies et al., NEJM 2025) 1,206 adults with type 2 diabetes, 68 weeks 13.7% vs 3.4% —
REDEFINE 4 (NCT06131437) 809 adults, 84 weeks, against tirzepatide 15 mg 20.2% vs 23.6%; non-inferiority not met (company announcement, 2026-02-23) —

Three points follow from the record rather than from opinion. The pair produced a large effect in its phase 3 trial; it also roughly doubled the rate of gastrointestinal adverse events against placebo; and in the one head-to-head trial, two peptides did not beat a single dual-agonist molecule. Each of those findings took a trial of hundreds to thousands of people to establish, which is the scale of evidence that no three- or four-peptide combination has.

What research blends contain

Blend (as listed) Peptides Composition stated on listings Registered trials
Wolverine 2 BPC-157 and TB-500 0
CJC-1295 / ipamorelin 2 Two GH secretagogues 0
GLOW 3 GHK-Cu 50 mg, BPC-157 10 mg, TB-500 10 mg (70 mg vial) 0
KLOW 4 GLOW plus KPV 10 mg (80 mg vial) 0

Listings read on 2026-09-23 for the GLOW vs KLOW page. No monograph defines any of these names, so each vial's label is the only statement of what it holds. The more peptides in one vial, the less freedom over each one: in a KLOW vial every unit drawn carries all four in a fixed 5:1:1:1 ratio by mass.

What a larger stack changes in the arithmetic

Each separate peptide is a separate draw unless it comes pre-blended, and each blended one is a fixed share of every draw. At 2 mL of water, 10 units of KLOW holds 2.5 mg of GHK-Cu and 0.5 mg each of BPC-157, TB-500 and KPV; any change to one amount changes all four. Separate vials keep each amount independent at the cost of one injection volume per compound. The arithmetic for both cases is in the blend calculator and the per-compound reconstitution calculators.

What this page does not do

It does not suggest how many peptides to combine, which ones, or in what amounts. It reports a registry count with its method, the approved combinations found and the stated contents of the blends on sale. The stacking evidence compound by compound is in the Stacking Guide, and each compound is covered in Medibact's peptide guide library. Decisions about combining any of these belong with a clinician.

Frequently asked questions

How many peptides can be stacked together?

No study sets a number. What the record shows: every approved peptide combination found is two peptides, every registered combination trial among the research-market compounds is two (cagrilintide and semaglutide), and no registered trial gives three. Research-market blends sold as GLOW and KLOW contain three and four peptides, and none has been tested.

Has anyone tested BPC-157 and TB-500 together?

Not in a registered trial. Of 101 studies returned for 14 research-market peptides on 2026-10-20, none names BPC-157 and TB-500 together. The combination is sold as a blend, often under the name Wolverine, on the strength of each compound's separate, largely animal evidence.

What is the most studied peptide combination?

CagriSema, cagrilintide plus semaglutide, by a wide margin: 31 registered Novo Nordisk studies in this count, including the REDEFINE phase 3 programme, kidney, appetite and paediatric studies, and three head-to-head trials against tirzepatide. It has not been approved yet.

Do any approved drugs combine two peptides?

Yes, two were found, both for type 2 diabetes and both an insulin with a GLP-1 drug in one pen at a fixed ratio: XULTOPHY (insulin degludec and liraglutide) and SOLIQUA (insulin glargine and lixisenatide). Because the ratio is fixed, the dose of one cannot change without the other.

Do peptides interact when stacked?

For the research-market peptides, no interaction study exists, so the record is silent rather than reassuring. The one research-market peptide with a combination programme, cagrilintide, has registered studies of how its blood levels and semaglutide's behave together and of interactions with atorvastatin and warfarin.

How many peptides are in a KLOW blend?

Four, as retail listings read on 2026-09-23 describe it: GHK-Cu 50 mg, BPC-157 10 mg, TB-500 10 mg and KPV 10 mg in one 80 mg vial. GLOW is the same without KPV. No standard defines either name, so the split is whatever a given vial's label states.
