Mazdutide is the first glucagon and GLP-1 dual agonist to reach a market: China approved it in 2025, and its whole phase 3 record comes from Chinese trials run by Innovent Biologics. That makes its dosing record unusually tidy — a small set of weekly doses, each reached through the same 2 mg first step. Medibact's mazdutide guide covers the molecule and its evidence; this page stays with the figures and the syringe arithmetic.
The trial doses, trial by trial
| Trial | People | Weekly dose and steps | Weeks | Result |
|---|---|---|---|---|
| Phase 1b, overweight (EClinicalMedicine 2021, PMID 34430840) | 36 | 1→2→3 mg · 1.5→3→4.5 mg · 2→4→6 mg, 4-week steps | 12 | −4.81%, −6.40%, −6.05% vs +0.60% |
| Phase 2, overweight (Nat Commun 2023, PMID 38092790) | 248 | 3, 4.5 or 6 mg | 24 | −6.7%, −10.4%, −11.3% vs +1.0% |
| GLORY-1, phase 3 (NEJM 2025, PMID 40421736) | 610 | 2→4 mg or 2→4→6 mg, 4-week steps | 48 | −11.00%, −14.01% vs +0.30% |
| Phase 2, BMI ≥ 30 (Med 2026, PMID 41875890) | 80 | 9 mg | 24 | −12.78% vs +1.80% |
| GLORY-2, phase 3 (JAMA 2026, PMID 42251595) | 461 | 9 mg | 60 | −16.65% vs −1.50% |
| DREAMS-1, type 2 diabetes (Nature 2026, PMID 41407859) | 320 | 4 or 6 mg | 24 | HbA1c −1.57%, −2.15% vs −0.14% |
| High-dose phase 1 (Diabetes Obes Metab 2025, PMID 40832785) | 32 | escalated to 16 mg, two regimens | 20 | −20.0%, −21.0% vs −0.1% |
Three things stand out. The first step is small: 2 mg in GLORY-1 and in the 6 mg phase 1b cohort, 1 mg in the lowest cohort. The steps are four weeks long wherever the escalation is published. And the step to 9 mg is not in the GLORY-2 abstract — the paper reports the maintenance dose and the 60-week result, not the path to it, so no step sequence for 9 mg is stated here.
What the side-effect rates look like by dose
The gastrointestinal rates climb with the dose. GLORY-1 reported mostly mild to moderate gastrointestinal events and discontinuation for adverse events of 1.5% (4 mg), 0.5% (6 mg) and 1.0% (placebo). At 9 mg in GLORY-2, vomiting was reported by 53.1%, nausea by 46.9% and diarrhoea by 39.4%, and 2.9% stopped treatment for an adverse event against 0% on placebo. In the 9 mg phase 2 trial, nausea reached 50.0%. The phase 1b report in overweight adults noted three mild, asymptomatic electrocardiogram findings in the mazdutide groups.
Units per vial size
Mazdutide is counted in whole milligrams, so the table works in mg/mL. Units on a U-100 syringe = mg ÷ mg/mL × 100. Vial sizes in this site's data are 5, 10 and 15 mg:
| Weekly amount | 5 mg + 1 mL (5 mg/mL) | 10 mg + 2 mL (5 mg/mL) | 10 mg + 1 mL (10 mg/mL) | 15 mg + 1.5 mL (10 mg/mL) | 15 mg + 1 mL (15 mg/mL) |
|---|---|---|---|---|---|
| 2 mg (first step) | 40 | 40 | 20 | 20 | 13.3 |
| 4 mg | 80 | 80 | 40 | 40 | 26.7 |
| 6 mg | 120 ✕ | 120 ✕ | 60 | 60 | 40 |
| 9 mg | — | 180 ✕ | 90 | 90 | 60 |
✕ = more than one 100-unit syringe holds. A 5 mg vial cannot supply a 6 mg or 9 mg amount at all. The pattern is the opposite of the microgram peptides, where too little water makes readings too small to resolve: here too much water pushes the higher trial doses past the barrel. How long a vial lasts is a division: 10 mg covers two and a half weeks at 4 mg, and a 15 mg vial covers one 9 mg week with 6 mg left. The mg to units page sets out the formula, and the mazdutide calculator handles any other vial and volume.
Where mazdutide sits among the dual and triple agonists
Weekly doses differ by molecule and are not interchangeable. The survodutide dosage page reports the other glucagon/GLP-1 dual agonist, titrated from 0.3 mg; the retatrutide dosage page reports the triple agonist's trial doses, up to 12 mg a week; and the GLP-1 dosage chart sets the approved and investigational drugs side by side. Mazdutide's 2 mg first step is also retatrutide's; the two ladders are compared in syringe units on mazdutide and retatrutide side by side. Against the FDA-approved dual agonist, the trial records and a 2026 network meta-analysis are set out on mazdutide compared with tirzepatide.
Mazdutide's two-step ladder against survodutide's eight-step one, with both sets of trial doses in units, is on mazdutide vs survodutide.
What is not established
Every published mazdutide trial was run in Chinese adults; how the doses translate to other populations has not been reported in the sources opened. No regulator outside China has reviewed a mazdutide dose, and the approved Chinese label itself was not opened for this page — the approval facts come from Innovent's 27 June 2025 announcement and the Drugs profile. The weekly half-life behind the once-weekly schedule is not stated in the abstracts opened. Whether a vial sold as mazdutide contains it, and how much, is not known without an assay. Nothing on this page is a dose or a schedule.
