A "Wegovy dose chart" is the label's escalation table plus the maintenance dosages that follow it. Since the 2026 label revisions it also has a seventh number, 7.2 mg, and a tablet ladder beside it. This page prints all of it from the WEGOVY label revised June 2026 and the OZEMPIC label revised May 2026, both read on DailyMed on 2026-10-06, and puts the trial behind the new top dose beside the chart. It reports the labels; it recommends no step and no dose.
The compound page with the trial record and research-vial arithmetic is the semaglutide dosage page; all the GLP-1 ladders in one table are on the GLP-1 dosage chart. The compound itself is covered in Medibact's Semaglutide guide.
The injection ladder
| Weeks | Weekly dose | Step size | Label status |
|---|---|---|---|
| 1–4 | 0.25 mg | — | Starting dosage |
| 5–8 | 0.5 mg | ×2 | Escalation |
| 9–12 | 1 mg | ×2 | Escalation |
| 13–16 | 1.7 mg | ×1.7 | Escalation; a maintenance option |
| 17 onward | 2.4 mg | ×1.41 | Maintenance (recommended for most indications) |
| 21 onward, earliest | 7.2 mg | ×3 | Adults, weight reduction only; maximum |
The label adds one instruction to the escalation itself: "If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks." The weeks are therefore the fastest path, not a fixed timetable.
The last row is the one most searches are about. It is the only step that triples the dose; every earlier step multiplies it by 1.4 to 2. The label permits it only after at least 4 weeks on 2.4 mg and only where "additional weight reduction is clinically indicated".
Maintenance by indication
| Indication (label) | Maintenance dosage |
|---|---|
| Weight reduction, adults | 1.7 mg or 2.4 mg (recommended); may increase to 7.2 mg maximum |
| Cardiovascular risk reduction, adults | 2.4 mg (recommended) or 1.7 mg |
| Weight reduction, ages 12 and over | 2.4 mg (recommended) or 1.7 mg |
| Noncirrhotic MASH with moderate to advanced fibrosis | 2.4 mg; may decrease to 1.7 mg, with re-escalation considered |
So 7.2 mg exists on one row only. For the other three indications the chart stops at 2.4 mg.
The trial behind 7.2 mg
STEP UP (NCT05646706) randomised 1,407 adults with a BMI of 30 or more and no diabetes, 5:1:1, to 7.2 mg, 2.4 mg or placebo for 72 weeks. Its registry entry describes a 20-week escalation for the 7.2 mg arm through 0.25, 0.5, 1.0, 1.7, 2.4 and 7.2 mg "every fourth week", the same 4-week rhythm the label uses, with 2.4 mg to 7.2 mg as a single step. The publication (Wharton et al., Lancet Diabetes and Endocrinology 2025, PMID 40961952) reports mean weight change of −18.7% on 7.2 mg, −15.6% on 2.4 mg and −3.9% on placebo; the difference between the two active doses was −3.1 percentage points. The FDA approved the 7.2 mg strength on 2026-03-19, as recorded on this site's semaglutide page.
Missed doses: the rule that resets the ladder
The Wegovy injection label's rule has three parts. One missed dose: given as soon as possible if the next scheduled dose is more than 2 days away; not given if it is less than 2 days away. Two or more consecutive missed doses: "reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions". The label does not say which lower dose. How far drug levels fall during one, two or three missed weeks is worked out on the missed GLP-1 dose page.
The Ozempic label is more permissive: a missed dose is given within 5 days, else skipped. The tirzepatide labels use 4 days and contain no restart rule at all (see the Zepbound dose chart). Three products, three different windows.
Pens, syringes and what they hold
| Presentation | Doses | Strengths | Volume per dose | Concentration |
|---|---|---|---|---|
| Single-dose pen | 1 | 0.25, 0.5, 1 mg | 0.5 mL | 0.5, 1, 2 mg/mL |
| Single-dose pen | 1 | 1.7, 2.4, 7.2 mg (HD) | 0.75 mL | 2.27, 3.2, 9.6 mg/mL |
| FlexTouch pen | 4 | 2.4 mg | 0.75 mL | 3.2 mg/mL (9.6 mg/3 mL) |
| Single-dose syringe | 1 | 0.25 to 2.4 mg | 0.5 or 0.75 mL | as the pens |
The 7.2 mg pen holds three times the drug in the same 0.75 mL as the 2.4 mg pen, so its concentration is three times higher. On a U-100 scale every single-dose Wegovy dose is 50 or 75 units of volume; no Wegovy device is read in units. Unit arithmetic for compounded semaglutide vials is on the semaglutide dosage page, and the reading of Novo Nordisk's dial-a-dose pens on Ozempic pen clicks.
Tablets, and switching between forms
The same label adds Wegovy tablets: 1.5 mg once daily on days 1–30, 4 mg on days 31–60, 9 mg on days 61–90, then 25 mg daily as maintenance. It states two switches. Injection to tablets: one week after the last 2.4 mg injection, start 25 mg tablets daily. Tablets to injection: the day after the last 25 mg tablet, start 2.4 mg weekly; if 25 mg tablets were not tolerated, the label says to consider 1.7 mg instead. Why an oral milligram is not an injected milligram is on oral semaglutide.
The Ozempic chart beside it
| Wegovy injection | Ozempic | |
|---|---|---|
| Start | 0.25 mg weekly, 4 weeks | 0.25 mg weekly, 4 weeks |
| Steps | 0.5 → 1 → 1.7 → 2.4 mg, 4 weeks on each | 0.5 mg; then 1 mg and 2 mg if more glycemic control is needed, each after ≥4 weeks |
| Maintenance | 1.7 or 2.4 mg (7.2 mg optional, adult weight) | 0.5, 1 or 2 mg; 1 mg in type 2 diabetes with kidney disease |
| Maximum | 7.2 mg | 2 mg |
| Devices | Single-dose pens and syringes; FlexTouch (4 × 2.4 mg) | Multi-dose pens of 2, 4 and 8 mg in 3 mL; single-dose syringes of 0.25–1 mg |
| Missed dose | Skip if next is <2 days away; restart ladder after ≥2 missed | Within 5 days, else skip |
"Switching from Ozempic to Wegovy" and "Ozempic to Wegovy conversion" are searched; neither label addresses either. What the labels do show is that the ladders are identical to 1 mg and separate after it.
What this page does not establish
No label states which lower dose to restart at after missed Wegovy doses, a conversion between Ozempic and Wegovy, or a schedule for anything above 7.2 mg. STEP UP's results are for adults without diabetes at a BMI of 30 or more. Every dosing decision on these products is a prescriber's.
