BPC-157 is a synthetic 15-amino-acid fragment of a protein found in gastric juice, studied for decades in animals, mostly by one Croatian laboratory, and sold on the research market as a lyophilized powder in 5 mg and 10 mg vials. No product is approved anywhere, so there is no label, no approved concentration and no labelled water volume. Its amounts circulate in micrograms rather than milligrams, which makes the syringe arithmetic the whole question: a 5 mg vial holds twenty 250 mcg amounts, and the water decides whether each one is 5 units or 15.
This page reports that arithmetic for both vial sizes, sets the figures that circulate beside the few human reports that exist, and says where each came from. It recommends nothing. The arithmetic in one line
Concentration in mcg/mL is the vial amount in micrograms divided by the water volume. Micrograms per unit is that figure divided by 100, because a U-100 syringe holds 100 units in 1 mL and each small notch on a standard 1 mL insulin syringe is 1 unit (MedlinePlus, "Giving an insulin injection", reviewed 2024-07-21). Units for an amount are the amount divided by the micrograms per unit.

No label or controlled trial supplies an amount, so the columns are the figures most often cited on clinic and vendor pages on 2026-09-18, 250 to 500 mcg per day, with 750 and 1,000 mcg added because the arithmetic is the same and the pages sometimes go that high. The pattern is the opposite of the milligram peptides on this site. A microgram amount never comes close to filling a syringe, so the concentrated end of the table is where the trouble is: at 10,000 mcg/mL, a 10 mg vial in 1 mL, 250 mcg is 2.5 units, which sits between two marks on a 1-unit syringe, and a one-unit misreading is 100 mcg, 40% of the amount. At 2,500 mcg/mL the same 250 mcg is 10 units and a one-unit misreading is 25 mcg, a tenth. More water gives a more readable draw; it never changes how many amounts the vial holds.
The 5 mg vial and the 10 mg vial
The two searched questions are what volume goes into a 5 mg vial and how a 10 mg vial is mixed, and the answers are the two rows above. A 5 mg vial in 1, 2 or 3 mL is 5,000, 2,500 or 1,667 mcg/mL, so 250 mcg is 5, 10 or 15 units; it holds twenty 250 mcg amounts at any volume. A 10 mg vial in 1, 2 or 3 mL is 10,000, 5,000 or 3,333 mcg/mL, so 250 mcg is 2.5, 5 or 7.5 units; it holds forty. A 10 mg vial in 2 mL and a 5 mg vial in 1 mL are the same solution, unit for unit.
Mixing steps, as labels describe them
No BPC-157 label describes mixing, because none exists. The steps that approved lyophilized peptide labels describe are consistent from product to product: the diluent is added to the vial, the vial is rolled gently rather than shaken, and the solution is inspected and used only if clear and free of particles (Egrifta SV label, DailyMed, revised 03/2024, read 2026-09-18). Storage after mixing
No label states a storage period for a reconstituted BPC-157 vial, because no label exists; storage after mixing is covered by Medibact's how to store peptides, which this page links rather than expands.
The circulating figures carry no source. The human record is three small reports in one journal: a retrospective chart review of 16 knee-pain patients given intra-articular BPC-157, alone or with TB-500 (2021, PMID 34324435); a pilot in which 12 women with interstitial cystitis received 10 mg injected around the bladder inflammation in a single procedure (2024, PMID 39325560); and a two-person intravenous safety pilot at 10 mg on day one and 20 mg on day two (2025, PMID 40131143). None used a subcutaneous microgram amount, so none supports the 250 to 500 mcg figure, and none was controlled. Four registrations name the peptide on ClinicalTrials.gov, the largest a 120-participant phase 2 hamstring-strain trial recruiting since February 2026 (NCT07437547).
The regulatory line moved in 2026. FDA placed BPC-157 in Category 2 of its section 503A bulk-substance list, the category for substances that raise significant safety risks, on 2023-09-29. A Federal Register notice of 2026-04-16 scheduled BPC-157 for the Pharmacy Compounding Advisory Committee on 2026-07-23, with ulcerative colitis as the use FDA evaluated, and on that day the committee recommended that compounding pharmacies be allowed to prepare it, along with KPV, TB-500 and MOTS-c; the votes are not binding and FDA decides (STAT News, 2026-07-23, read 2026-09-18). None of that supplies a dose.
